Hello,

I am getting ready to fill out the I-485 form for my husband who just got his visa, but I honestly have no idea what to fill in for some of the questions that are on the form. Is there anyone that has completed this form along with the travel and work permit forms who would be willing to be in contact with me through email to help me out when I have a question about the forms? It would be greatly appreciated if someone could help out that way, if not than I can just do a couple more postings on here and get help from everyone. I would like to know if my new last name is required on the forms since I just got married to my husband or may I use my birth last name? It might not even matter but I thought I would ask. I hired a lawyer for the first process of the visa and it all went well but most of the information she would give me was always incorrect and I would end up looking else where for the information. I decided not to use her services for this next process and try to do the rest on my own.

Use your legal name on the forms, if your name changed due to marriage use that name. There is usually a section for other names used where a maiden name would go.

Thank you for this information. Now for the I-693 my husband got his medical examination done in Ciudad Juarez, Chihuahua does this mean he does not have to get this done again? I read he wouldn't have to again if the paperwork is filed within one year? Is this correct? Thanks for your help?

I-693 must be performed by a US civil surgeon. The document is sealed and must remain sealed when submitted.

If it was less than a year ago, he won't need a full medical just a transcription of vaccines. If vaccines are missing, he will need to get those first. We went to the local health dept. which is usually much less expensive than a doctor. Then we took everything to the civil surgeon and had the information transcribed for $25. Many civil surgeons will want to charge more and insist on a full medical, so call around first. Good luck!
