If you have sex with someone you are dating, romantically interested in, in a serious/casual relationship with - you will care about the pleasure of your partner as part of the act, because if you don't then it isn't very likely that you will be having sex with them again.

Conversely, there are very few situations where the guy would not care about the partner's pleasure during sex.

1) You are having sex with a prostitute - you are paying for your own gratification.

2) You are violently raping a woman - the act is all about the gratification of the rapist. That he might injure the woman's genitals as part of that doesn't enter into it.

3) A guy who has sex with a drunk woman passed out at a party doesn't care if she enjoys the sex because she is unconscious and the act is once again all about his own gratification.

There's a big difference in being bad at sex and simply being indifferent to your partners pleasure.
