I don't know anything about this form. My fiancee had a medical check in Japan. She may need some more vaccines...I don't know.

Where do I get the form that we need to send in with our AOS? Do the Civil Surgeon have it? I couldn't find it on the USCIS website...

My fiancee wasn't given any forms along with her visa...just her x-ray.

I don't find the FAQ at the top of the forum useful...But if there is anywhere else where I can get the answers, please guide me!

Sorry I found it on the USCIS website by searching on Google. Thanks but I am still really confused.

What do we do with this form? I really don't want to have to do another medical check. How do we get the doctor to fill out the form without doing that though?

We don't have any proof of doing the medical check in Japan because we submitted that to the embassy and never got it back...Will we get it back once she comes to the US or something?

Please just let me know what we should do with the form.

Regardless, the Civil Surgeons here in the US are the ones who have the forms available. When your fiancee needs to get proof of her vaccination history she goes to a Civil Surgeon here in the US who already has the form. She needs to bring proof of whatever vaccinations she has had with her to her appointment - and if she needs more here try to get them done at a Health Unit before hand as it will be cheaper - and he/she will transfer the information to the relevant part of the I-693. He/she will then seal the form in an envelope (request a photocopy for your records first) and you would then submit the envelope with the AOS application. You should never actually need to have a blank copy of this form, although it certainly doesn't hurt to take a look at it.

Translate them into English if you have them

if not then the CS is prepared to handle this situation, sometimes via a "titer" test

And as I look at the I-693...it has a chest X-ray...how do we prove that we've gotten that already?

Sorry, I just know how expensive doctors are so we don't want to get stuck paying a ridiculous amount to have them do it over again.

They gave her an X-ray...It probably has her name on it doesn't it? That makes sense...but if it doesn't, there would be no way to prove that it was actually HER x-ray.

best to call around then and see how the CS wants to deal with your situation.... they are the ones who will decide.... just make sure they dont talk you into something you dont want or is needed

I think that the Xray is only required if someone "fails" the TB skin test.

I didn't have ANY immunization/vaccination records on me, didn't bother asking my mom, let alone translating :P

Just went to a CS and he gave me all I needed except the ones where I'm too old to have.

Then he also gave me a TB skin test, and blood test.

All for $300-ish. Wasn't too bad. Done in 3 days, had the sealed envelope and everything. :)

As a K-1 she doesn't need anything other than proof that she has now met the vaccination requirements to submit with the AOS application. The K-1 medical did provide all of the other necessary information and the copy of the medical information is now in the hands of USCIS - it was contained in the yellow/brown envelope your fiancee turned in at the POE. K-1 applicants, however, are not required to have all of the immunizations in order to obtain the K-1 visa as it is not an immigrant visa but a non-immigrant visa. To adjust to immigrant status (AOS) the immigrant now needs to prove that they do have the necessary immunizations. For many applicants they will have had these completed for the K-1 medical as well, but some countries don't have certain vaccines available, so individuals from these country need to complete their vaccinations in the US. So, unless specifically requested later on by USCIS to submit a complete immigration medical (which means they have lost the one submitted with the K-1 envelope) just worry about getting the vaccination history transcribed and don't re-do the medical.

If the documents verifying her vaccination history are in Japanese, yes, get them translated. If she still requires some vaccinations then get them done before she goes to the Civil Surgeon and bring proof that she has had them with her to the appointment. If there is a 'sequence' of vaccinations required she doesn't need to have completed the sequence, just be in process of completing the sequence. The remainder vaccinations in the sequence will be considered medically inappropriate at this time and therefore not necessary for the AOS application. In her own best interests, however, she will want to complete the sequence.

All the Civil Surgeon needs to do is to transfer the vaccination history from her documentation to the I-693 form. Don't let a Civil Surgeon talk you into re-doing the whole immigration medical either. It is NOT needed - and you can print out the statement on the AOS application stating that K-1s only need the vaccination part of the I-693, not the whole medical, and bring it with you. If the Civil Surgeon insists on re-doing the medical, then find another Civil Surgeon who is willing to provide only what is required and not try to make extra profit at your fiancee's expense by doing unnecessary services.

If she has vaccination records, transcribe it for I-693 :star:

But since Japan stop giving a shot 'MMR'(Mumps,Measles,Rubella) I guess she may need it done in USA.

And also may need booster shot of 'DTap' or 'Tdap'(Tetanus,Diphtheria,Pertussis) (If her record is old).

I had these shots done at county health center in San Diego,cost me &10 for all.

VERY IMPORTANT::: don't get the form from the doctor. go to the USCIS and download the newest form. If the doctor has the old form he will required you to get a HIV screening which is no longer needed. This is not a requirement on the new form. Then print out the attached file which is the civil surgen instructions. Near the last page are the required vacines depending on age. Very important reading because this process is subjective and doctors, if not ethical and educated, will take advange of you if you don't know the rules.
