Right ok thanks, now on my DS-3025 most of the ticks are in the "Not Age Appropriate"

So i assume, I can just give this to them, they copy the info across and i don't have to have any more injections, is this correct?

UPDATE:

Ok so i have now got the form DS-3025 which shows that I have had the MMR last year and the rest are ticked in the N/A collum. Where do I go from here, do I still need to get a CS to fill out anything on the I693 etc????

You still need a CS to sign off on the I-693. The CS would only have to transcribe your vaccinations to the worksheet. Some CS's will just charge a nominal fee for the transcribing BUT in most cases want to collect on the visit. If there is only 1 CS in your town most likely he/she is cashing in on it. Some people have mailed in only the DS-3025 and never received an RFE however if you want to take that chance and they issue a RFE then there could be a delay in processing your AOS.

He does not NEED a CS (civil surgeon) to sign off on his DS-3025 if down the bottom of the form the box "vaccinations complete" is ticked. Out of interest Astrid from Australia sent only her DS-3025 and it was marked complete. She has her greencard and never received any RFE's.

That said, if it makes you feel better to have a CS transcribe it, do so. My vaccinations were complete but my doctor ticked "vaccinations incomplete, waiver may be granted" or something like that. Because of that little "mistake" on his part I had to pay $50 to get a CS to transcribe the vaccinations over.

No, down the bottom the box ticked is the one that says....

Applicant maybe eligible for blanket waiver because vaccinations not medically appropriate.

In that case then you DO need to see a CS. Sorry.

Hi, i just received interview letter today for my AOS. I didn't include the I-693 on my AOS application, i just mailed it with DS-3025. I did not get any RFE. But, my question is.. on the letter it says i have to bring a complete medical examination (I-693) and vaccination supplement in a sealed envelope (unless already submitted)

So, it means i still need to go to a civil surgeon for my I-693 to bring for my interview??

You don't need a Civil Surgeon to transcibe your vaccines onto the I-693, unless some are outstanding (like two are for me) and you get them done after the medical but before AOS. A CS matches up all of the paperwork, thats what the I-693 is for. If all is in place at your medical and they told you you're all up to date, then no CS is needed. You just submit you DS3025 worksheet with your application for AOS.

However, many people have reported their medical paperwork going missing within the system so people have been recieveing RFE's when they haven't submitted the I-693 - even if they didn't need one. Thats beacuse your application for K1 and the application for AOS are two separate depts and they are not matching them up properly. So its up to you whether you want to go the extra mile and do it for them to avoid the RFE.

According to the USCIS, submitting the DS-3025 with the application is not sufficient. The DS-3025 needs to be in the POE packet from the consulate for it to work
