Hi,

I was wondering what sort of documentation everyone uses for proof of insurance. My husband has been added to my vision, dental, and health insurance, but all of the insurance cards have my name only along with the status of +1 or family on them. Of course, my health insurance doesn't like that there hasn't been a SSN listed for my husband, but that's b/c he doesn't have one yet and won't until he receives his GC. Right now, the plan is to print off pages from my online account information with each insurance and bring them with us to the interview. But I wanted to see if anyone else had a better idea.

If you dont have a card with his name on it maybe your HR department can give you a copy of the application where you added him to submit with a copy of the card.

Nik had insurance cards with his name on them, so our experience doesn't directly translate.

However, they have accepted other evidence that I had printed off (paystubs in particular) from the company website. If you can print off the pages which show him as the beneficiary for your work related things (401k too?) including health insurance, this will go farther.

Maybe also show that you have made him into your emergency contact, etc.

Also, the whole evidence of relationship thing isn't a checklist - have everything or die, but rather the community here has compiled a list of possible pieces of evidence, the preponderance of which is used to make a decision. If some of them don't work out, it shouldn't be a big deal - just so long as this wasn't the ONLY piece of evidence you had.

Thanks for the suggestions! I knew it was too much to hope for one piece of paper proving everything. :) At this point, I'll bring my paycheck stub(which shows I pay for family health insurance), the insurance cards, and printout from each company with both our names.

As a side note, it turns out Capital One credit card allows people to add a person without a SSN as an authorized user. We can't get a joint bank account any other place than Bank of America(hate them!) without an SSN, but credit cards will sure allow to another user in the hopes of you running up debt. :)

Keep in mind that in most instances, as soon as the work authorization is approved, you can go to the Social Security Administration in your city and apply for a SSN. Since the work auth is usually approved at least several weeks before the AOS interview, you may have time to get the SSN on your insurance, bank and credit accounts, etc.

As far as insurance goes, do you or can you have online access to your benefits? I know my UHC is available online and has all the member info. Just a thought... Good luck!

For adding your husband to your health coverage you would have had to provide the information to your employer. If you didn't keep a copy of the information or receive anything back confirming the change/update to plan, then ask your Human Resource person either to provide you with a copy of the information you submitted or a statement on appropriate letterhead verifying that (husband's name) has been added to your health insurance plan as your husband and receives XXXX benefits. You could show on your pay statement where the 'family' or 'married' option premiums are being deducted from your pay as well. If your online account gives the 'family' information checked off and lists the name of your family member anywhere, then that would work as well. You could get any or all of the above type of documents as evidence.
