I believe you still need to send in the I-693A form. If you have the vaccination worksheet, you can take it to an approved Civil Surgeon and have them transfer the information to the I-693A form for a minimal fee. Don't let them tell you that you need another medical exam, because K1 and K3 applicants don't unless their medical is 1-year expired.

If you choose to just send in the Vaccination Supplement, you may get by without an RFE, but you would still probably need to bring the I-693A form to your interview. I think sending in the form also increases your chances of being transferred to CSC and approved without interview.

Better safe than sorry. Some have gotten RFE's for the I-693A and some have not so it's better to just go ahead and get it. You may need it anyways at the interview if you did not send it in.

If the DS-3025 shows that your vaccinations are complete, they will not require an I-693a. You can send a copy of the DS-3025 with your AOS package to be sure they know your vaccinations are complete, but they should have the package you turned over at the POE for the interview and the DS-3025 is also in there.

The I-693a is required if your vaccinations were not complete at the time of the medical and you had to complete them for the AOS.

If I were you I would get the I693A form filled out and send it instead of the DS-3025. Sometimes they have accepted it but sometimes you get an RFE and to be safe I would go with the supplement form because this is what is in the instructions to send. If it is not too much trouble or money than go with the I-693A form.

Good Luck

The I-693a is required if your vaccinations were not complete at the time of the medical and you had to complete them for the AOS.

Unfortunately I don't think there's any guarantee you'll escape an RFE even if your DS-3025 shows that you have had all required vaccinations. See the link below for a recent post from Debby, who was given an RFE at her AOS interview to submit I-693A. She had sent DS-3025 in with her AOS package, and it sounds like all her vaccinations were complete.

http://www.visajourney.com/forums/index.php?showtopic=24729

But on the other hand, there are plenty of folks here who submitted DS-3025 in lieu of I-693A and got through AOS without any vaccination record related RFEs. So I suppose it depends on how risk averse you are. In my case, we went ahead and went to a civil surgeon to get the I-693A. Perhaps just as well, since my husband still needed three shots that weren't available in his country.

MAKE SURE you do the I-693A and pay the money to have it transferred. ALL my immunisations were completed and as I am from Australia there was no mix up with understanding etc.....however it caused an RFE at the interview. Here we were thinking that we had EVERYTHING we needed and then BOOM at the interview stage. We spent $75 for the I-693A to be completed and never even saw the doctor. I have read of people not having the I-693A form and having their AOS's approved but trust me when I say DON'T RISK IT. Before my interview I didn't know that I had missed anything now here I am not knowing once again what is going on with our case. We did have the I-693A in the mail the same day as our interview though.

Not worth the risk. Get the vaccination supplement.

You might get away with it, we did.

We just sent the DS form, then took the I693A to the interview, which it says you can do on the AOS interview letter. At the interview I offered it to the officer, as he didn't ask for it....his reply was no we've got it!!!

Well I wasn't gonna argue with him, but no they have never had an I693...only the DS form.

I-693A in a sealed envelop is required for AOS

Wes' was not in a sealed envelope (the doc didn't put it in one) but it didn't cause a problem for us. We did include the receipt from the doc's office in case USCIS cared to check the authenticity of the document. I'm not sure all the docs know they should put it in an envelope.

Ditto to above. - REQUIRED!!!

Look up a local "medical examiner" or "examiers" and ask them if they can transfer the records over. We were able to do it but had to pay the 30ish fee to get it done.

just because some ppl get away with it, doesn't mean you definitely will. what officers let slide is up to them... total crapshoot.

i wouldn't risk an RFE for something like that. a doctor can fill one out in minutes and if you are lucky you can get it done cheaply.

mine was $35... if i'd skipped it and got an RFE, the days or weeks lost for the delay would far surpass a few measly bucks.

but, its totally up to you if you feel like taking that chance. you might be one of the lucky ones. *shrugs*

good luck :star:
