Just wondering...do we call a USICS civil surgeon on our own when we are ready to file the AOS, or do we wait for USCIS to tell us to do this...or what?

You will need to include an I-693 medical report, sealed in an envelope by the civil surgeon, with your AOS submission. So you need to call them and get that done well before you send off your AOS packet. It is not too soon to start calling around your local civil surgeons (you can get a list of them on www.uscis.gov) to get your vaccination results transcribed to an I-693 as soon as possible.

You will need to include an I-693 medical report, sealed in an envelope by the civil surgeon, with your AOS submission. So you need to call them and get that done well before you send off your AOS packet. No. You do NOT need to include medical report. K1's have a medical that is valid for a year. If you vaccinations are COMPLETE on entering you can send just the DS-3025. HOWEVER (as said in the thread above) some people have encountered issues where their medical cannot be found and need to do a medical again (or find the old one), or where they THINK their vaccinations are complete when they aren't.

I personally had my shots completed but my form has the box "immunisation history not complete" ticked because some weren't age appropriate. For this reason I went to a CS and had my VACCINATIONS ONLY transcribed into the I-693. This means the CS will fill in part 1, part 2 (vaccination table only) and part 5 and seal it in an envelope for you.

Procedures appear to have been in flux about whether you need I-693 Parts 1,2, and 5, or whether you can get away with just the DS-3025. Amongst the October 2009 AOS filers, a small percentage (about 10%) sent only the DS-3025, and were mostly approved without problems. However, in the next few months, more and more people sending just the DS-3025 began receiving RFE's (Request For Evidence), delaying their applications and causing undue stress and confusion.

V&T are otherwise completely correct. As a K-1 entrant, you do not need a full medical exam, and you do not need the entire I-693. Theoretically, according to the USCIS instructions, you should be able to get away with just sending a DS-3025 if your vaccinations were up to date at the time of your medical. But it seems to be a safer and more reliable practice to pay a nominal fee (not more than $50. Phone the CS's in your area until you find one that will do the vaccine transcription without requiring a full exam) and include the I-693 Parts 1, 2, and 5. in an envelope sealed by the CS with your AOS packet.

That's the instructions that I was following. I included it.

Now that I received the initial interview notice, and it says, Bring a completed medical examination (Form I-693) and vaccination supplement in a sealed envelope (unless already submitted).

It confused me at first, and then figured that some people might've sent in a copy of it, along with copies of other documentation.

And it also says, Bring Original and copy of each supporting document that you submitted with your application. Otherwise, we may keep your originals for your records.

It took only two (2) months to be approved. Hooray!

December 5, 2009 - I-485 application was submitted.

January 12, 2010 - I-485 application was re-submitted due to address error.

January 20, 2010 - I-485 application receipt notice arrived.

January 25, 2010 - Biometrics notice arrived.

February 9, 2010 - Biometrics taken at local USCIS office.

March 5, 2010 - Initial Interview notice arrived.

April 15, 2010 - Employment Authorization Card arrived out of blue.

April 29, 2010 - Initial Interview scheduled and... APPROVED!!! :) Conditional GC will arrive in 2-3 weeks.

April 30, 2010 - SSN reactivated. Newly printed card will arrive in 3-5 days.

May 7, 2010 - Welcome Notice arrived! Hooray! :)

May 8, 2010 - Freshly printed SSN card arrived.

May 26, 2010 - Conditional GC issued.

June 1, 2010 - Conditional GC ARRIVED! Yippee! :)

June 2, 2010 - SSN upgraded. Newly printed card will arrive in a week.

June 9, 2010 - Freshly printed SSN arrived.

October 10, 2011 - I-751 petition was submitted.

October 18, 2011 - I-751 petition was returned. It turned out to be USCIS's error sending it back to me.

October 19, 2011 - I-751 petition was re-submitted.

October 26, 2011 - I-751 petition receipt notice arrived.

November 18, 2011 - Biometrics taken at local USCIS office.

December 27, 2011 - I-751 petition Approval letter was issued.

December 28, 2011 - I-751 petition Approval letter arrived

December 29, 2011 - GC was issued.

December 30, 2011 - GC arrived.
