There are two barcoded sheets - which one do I enclose and do I enclose it with AOS and DS3032 - sooo excited and overly confused!

Yes. The one with ATTN: CMR AOS, enclose it with your AOS packet and put it on top of your forms/documents, while the other cover sheet with ATTN: CMR, include it in your beneficiary's DS230 packet (again put it on top of the documents) when you send it to NVC .

Anyway, you can see those barcoded cover sheets for yourself when you pay the AOS and IV fees online, and you can print it after it showed PAID

And... when I got home... *giggle* There was the e-mail telling me to pay the $400 IV bill and send in the DS-230. Thank you, NVC. Only about a week late.
