My fiancee is in the United States and we will soon be getting married and applying for AOS.

I see now that I-485 required a "Vaccination Supplement." I have no idea where I am supposed to get this, or even what form it is. Obviously, in order to get the K-1, my fiancee had a medical exam in her home country. The doctor gave her a package that she gave to the embassy during her interview, and they took it and gave her the "DO NOT OPEN THIS ENVELOPE" envelope that she was only supposed to give to the Customs and Border Protection agent at her point of entry. She did so, they took the envelope, and admitted her.

I suppose that the Vaccination Supplement was taken by either the US embassy or, more likely, Customs and Border Protection at the airport, but how in the world am I supposed to get it back from them in order to submit it with her I-485? I must be missing something here.

I'm in a total panic about this mysterious supplement so I would appreciate any help.

hi Mike don't be panic. Did your fiancee have the vaccination copy for her? the DS 3025 something like that if not mistaken.

I know St.Lukes gives copy of the vaccination records.In that form it shows which vaccine is needed to be completed here in US and USCIS provided list of Civil Surgeon to interpret it. Just look in the uscis.gov site and look for civil surgeon. When you get there it will ask for your zip code and then it will give you list of civil surgeons located in your area. Then you can call the nearest or all of them to see if who's the cheapest. If you call them just tell them that you only need her vaccination completed since she had her medical exam overseas already. And as long as the med exam overseas in not yet one year ago no need for another medical exam.

Also you can print the form I 693 from the uscis.gov site also under forms.

I don't think there ever was a copy for her. She was given an envelope and told to turn it into the embassy, which she did. Then when the embassy gave her her visa, she got the envelope that is turned into CBP, which she did. As far as I know neither one of us has ever actually seen her filled-out form.

She was told that she did not need any vaccinations to be done here because she had them all. I have no idea how I can prove that now that I do not have any form at all from the doctor.

well in this case she needs to ask a copy of her vaccination records from her country. that's the only proof to show that she had all her shots. you think she can call the place where she took her med exam? and ask them to send a copy?

Apparently she had forgotten that they gave it to her. However, it's a fairly low-quality photocopy -- no one would mistake it for being an original. Is this what they are looking for? Is this junky copy good enough?

Yes it's perfectly fine. it's more of a "reminder" that they had the shots done.

She MAY get an RFE if she doesn't complete the I-693. Read this thread: http://www.visajourney.com/forums/index.php?showtopic=101202 for more info.

Yes thats what they are looking for. It may look junky but still just made a photocopy for you to send for USCIS. Keep the original. And make sure it says in the worksheet that the required vaccinations are really complete. ^^ Glad to know she have it.
