Hey all, I just got back from the medical center as we read on here that before we file for the AOS that we have to get my vaccination records transcribed onto a certain form. I get there, and give them my records, and after a very long wait I am called up, given an eye test (which i thought was weird) and had to give a urine sample.

I am then told that I have to have another medical. I told them that I had already had my medial and didn't require another one, and only needed the form regarding my vaccines.

The very rude Doctor decided she was correct that I HAD to have another medical etc, but I refused to have another one since I was not going to pay any more money on a medical I didn't need.

I left there with only the vaccination part of the I-639 complete. My vaccine records are up to date and I have them all.

Is this all I need to send off? I have never been given a copy of my medical ever, I thought the immigration office already has my medical in that packet I surrendered at my point of entry.

Will the two meet up eventually? I just paid over two hundred pounds in the UK for a medical so I don't want to pay for another.

We only submitted the vaccination part of the I-693 too (along with the Part 1 (Information About You and Part 5 - Civil Surgeon's Certification) because my vaccinations were up-to-date and my overseas medical is still valid. There are people who have received RFEs for the I-693 though. Apparently, they sometimes lose your medical forms/results from the overseas medical so they send an RFE requesting a full medical to be recorded on the I-693 :(

You didn't state that you had an RFE or anything so I assume you mean you're about to send of AOS and are getting all the stuff together.

You are right, you DO NOT need another medical as long as it's been LESS THAN a year since your previous medical.

The vaccination part complete is fine AS LONG AS she also signed part 5 (and you have completed part 1). If part 5 was not complete then no, the form is not complete (if would have been given to you in a sealed envelope).

As an aside, you DO NOT HAVE to have your vaccinations transcribed into the 693 form if your vaccinations are completed during the K1 medical, but some people think they're complete and they aren't so they get an RFE. If she gave you shots at that doctors office to "complete" you then your K1 vaccinations weren't complete and you will need to get her to sign off on Part 5 of the 693 form.

You said "is this all you need to send off" but no it's not. if you're sending this as part of an RFE then you should send the RFE with the sealed envelope (keep a copy of the RFE for your records). The reason you send the RFE with it is so they can put it with your file wherever it's been stashed while waiting for you to respond to the RFE. If you're sending it with the AOS packet then obviously it's with the AOS packet.

If you send the form by itself, without the AOS forms, or an RFE (like if you forgot to include it and you're trying to make sure you DON'T get an RFE) it's likely it will get lost in the big abyss and you'll just have to do it again because they lost it somewhere.

**Edit - if you get an RFE for the medical and it's been less than a year since your K1 medical, then SOME people have simply contacted their K1 doctor to get a copy of the medical sent to them so they can RESEND it to them as a note "medical completed for K1 visa" so they didn't need to pay for another.

Fatima's right, you need part 1 (that's done by you) and part 5 as well as the vaccination section. And it has to be in a sealed envelope. Did they do all that?

I hear you about not wanting to redo the medical, and basically throw out the extortionary one we've already completed!

If you have a sealed envelope, and you aren't sure what's inside, I might try sending that in. Also send a copy of the DS3025. Some people have had marginal success by including a copy of the instructions for K-1s to give the adjudicator a hint. Know that sending this in might put you at risk for an RFE, but that is easily answered if you don't mind the delay.

Alternatively, if you are sure you don't have part 5 or you don't know what's inside the sealed envelope and don't want to risk an RFE, you should get the transcription done at a different office. Call around and find a CS who is more amiable.

She gave me a copy of what she did, and it looks like I have part 1 completed, which I signed. I also have the sheet with all my vaccine records, and she filled in part 5. She also put the original copies in a sealed envelope saying for USCIS use only.

Thanks everyone, hopefully they wont lose my medical work. How many people get theirs lost? Should I already apply for another copy just in case?

No, it sounds like you are good to go.

Put that sealed envelope, and the highlighted instructions (optional), and a copy of your DS3025 from your visa medical in there, and you should be set.

If you get an RFE, it is either wrong or because they lost the whole medical, and we can help you work through that IF it comes.

Yup! You're good to go!

IF you do get an RFE for the I-693, don't worry. Like Nik+Heather said, we'll be able to help you through that!

Hopefully we won't get RFEs though! I sent a cover letter with the I-693 too - with the I-485 and I-693 instructions regarding the K-1 medical, just in case (also included a copy of the DS-3025 Vaccination Documentation Worksheet AND the receipt from my overseas medical, LOL)

One thing with the ds-3025, when my medical took place, there was an error on my NHS vaccination record saying that i had only had one MMR and one MR, but now I got it sorted proving that I have had both my MMR jabs and am up to date on the form she did today.

I take it that if I just note that in my cover letter etc then that will all make sense to the person reading it all right?

They won't be surprised that the vaccination record by the CS is more complete than the vaccination record by Knightsbridge. Probably not even worth a note about it, but do what makes you feel most comfortable, I think.
