We only had to fill out 1.

They created a new one in June 2009.

The new form only has one sheet, correct, however four copies should still be sent in with any package you are filing.

We only sent 1 for each of us and we are waiting for an interview.

The old forms has specific addresses on them and were sent to specific areas. I am thinking they just got better at sharing the info.

*shrug* Then more power to you. I certainly don't think the extra three sheets would hurt any, but I suppose that's just me.

I only sent 1 copy and was approved. It never came up at the interview. I'm pretty sure only sending 1 would be fine, but if you want to be safe then I guess sending an extra 3 couldn't hurt.

The new guidelines state only 1 copy - actually I think that has been effect for a while - here is another recent thread about it:
