We also did not go to a civil surgeon, but I did attach his medical to a signed I-693 by my SO. I also attached an explanation on my cover sheet and on a cover sheet for the I-693. I would think that at least you should sent the I-693 with just part 1 filled out with a copy of the medical attached, but we will see how it will turn out! I wish everyone a speedy AOS!

We are on #20 of the flowchart (K1-VISA) and we have read the guidelines but still a bit confused. We already applied for SS#, got married and I (petitioner) have done my change of name.

Our package includes:

I-485

G-325a- Do we need this since we already completed this when we initially applied for the KI-VISA?

I-765

I-864

I-693- Not sure if we need this again because he has already done the first stage of his medical examination after I-129F was approved before his interview in Lagos

Are we missing anything or are all these forms required in the AOS Package?

We live in Maryland, do we mail it to Baltimore since we are close to Baltimore or Chicago Lockbox? If Chicago? Which address? I read somewhere that you can mail AOS to the nearest processing center (not sure if I misread)?

Do we send all these forms at once?

Please advise, thank you as always

Yay! That's a quick turn-around!

Jolex, I don't yet have the AOS form list as memorized as I did for the I-129F & interview... but I followed the Guides (linked at the top of this website) and I remember seeing the Chicago address in the AOS I-485 instructions file.

All K-1 based AOS filings go to Chicago. You'll want to make sure you read these http://www.uscis.gov/files/form/I-485instr.pdf because it talks about evidence to send with the application.

Nat, how did you sign up for the emails? Did they have it from your K-1 application? I didn't see a spot to add emails on any of the AOS paperwork.

Do you get three individual case numbers (one for I485, I131 and I765), or is there just one number for the entire file? I got a number on the back of the check...but the status check online is not recognizing the number. Does it start with MSC?

Thanks Nat. I was starting to get anxious that they sent the entire case back because I didn't include the I-693...LOL It was such a relief to see that check cashed tonight...we were both so happy. So the number on the back of the check is probably for the actual I-485. Well let the journey begin!

You will get 3 case numbers. Add 1 to the number at the back of your check - that should be your I485 case number. Yes, it starts with MSC.

THANKS! We got them in the mail Saturday, and you are spot on about adding 1 to the number on the back of the check. The other two were not consecutive numbers with the one for the i485. We are officially waiting on the red tape process now. LOL
