Yes. The USCIS will only accept an I-693 signed by a Civil Surgeon.

Is a civil surgeon really needed to certify the I-693 medical exam form? My fiance had her medical exam as instructed prior to getting her K1 visa and we no longer have access to her medical documents.

It would be a requirement if she needed the medical done again. The medical she has already done is good for a year & becomes part of the file that is reviewed for the AOS intervew.

Can you, by any chance obtain a copy of it from the medical center itself?

There should have been a big brown envelope with paperwork in it that your fiance handed in when she entered the US. That is where her medical examination details are. They should already be with USCIS waiting for your to send the I-485 so that they can be compiled together in your file. I know that they ask you to send a copy of the paper you received from the doctor, but I'm thinking that as long as USCIS has her brown envelope, you may be able to just include an explanation when you send your AOS file....If they do not have the papers, they will let you know..but I'm sure they'll have them.

We met in October 2007 and our immigration journey started in July 2008 when we filed for the I-129F Fiance Visa petition.

Please review the guide for AOS which has recently been updated. Assuming your fiancee's medical was done less than a year from when you'll file for AOS, you don't need to do another one. A civil surgeon needs to fill out the I-693 vaccination section, and sign the last page. Your fiancee fills out the first page, "Information about you," and the surgeon will seal it all in an envelope for you. Many civil surgeons will try to convince you she needs another medical. Don't fall for it.
