FEE on transferring vaccination info on I693a?

hi there..guys, i'd like to ask how much does it usually cost to have the vaccination info transferred to the i-693a form? i've called all the uscis authorized civil surgeon here in pasadena area and they charge from $95-$120 ( no vaccines included in those prices ).

do i need to have another vaccination for MUMPS (or MMR) & VARICELLA

& do i really need to get TD(tetanus-diphtheria) vaccine since this wasnt available in the phils?

can anybody suggest a doctor that charges much less?

I guess we were very lucky to find a civil surgeon in the Seattle area that only charged us $35.00 to transfer the information from the vaccination worksheet onto the vaccination supplement.

You will probably need to get the Tetanus vaccine now that you are in the U.S.

Hi, 1st you need to get the Td shot, i got it in local clinic here and its free. Varicella is needed only when u didnt show any proof in St. Lukes Phils. that you'd experienced chicken pox already. MMR#1 shot you had in St. Lukes last time, and you will have 2nd shot of MMR when ur in US "ONLY" if your not immune and need the 2nd shot.

And transfer fee of vaccine in form I-693A in Atlanta only charge me $20. I don't know the case in Pasadena, better try to serch in USCIS website the "lists" of recognized civil surgeon and call those clinics that are nearer to you.

In my case, i choosed 10 clinics to call, and 2 out of 8 clinics, had the price of $20, some asking for another medical exams. In my 2nd MMR, i choosed to have a bloodtest (titer) i ahd it free under our family health insurance coverages and showed results that i am immune and doesnt need for a 2nd MMR shot. Vaccines shots vary in prices between $50-100+.

Las Vegas:

$50 TD shot

$20 Fee for I-693A Vaccination Supplement

As others mentioned, just contact the USCIS-approved Civil Surgeons in your area and shop around. From what I understand, many doctor offices are not fully competent/aware of immigration law and will try to charge for an entire medical exam. Since you've already completed the exam at Manila SLEC, you only need the TD shot and the supplement form filled out.

She had all of her shots. Doc here in Cincy charge $15 and filled the I-693 out... no problem!

I had to call around a lot, after being refused by 7 doctors I finally found someone willing to transfer the information for $ 65. :angry:
