We just received our appointment date for our AOS.....but it says we also need a form, I-693. Now, I don't understand. Before she even was able to come into the United States she had to go to a US approved doctor in her country and do basically the exact thing that it is asking us to do, and pay for, again. This seems a bit pointless to me. Why would she have to prove all of this in the beginning and now prove it again? Am I missing something or do we in fact need to have her taken to a doctor here with the I-693, and if so wouldn't this theoretically render the previous forms meaningless??? Thanks for any info!

The only thing we needed was the vaccination supplement given at the medical exam in Kiev, provided the medical was within one year. If your fiancee took her vaccination records to the medical exam to be transcribed onto the DS-form, that should be all you need.

You should have received a copy of all the immunizations from the doctor/hospital when your fiance had the exam. Just submit that.

Some people get away with just submitting the DS 3025 vaccination record and some don't.

The requirement is anyone that has had a immigration medical within the last year only needs to have a USCIS designated civil surgeon transfer the vaccination record to the I-693. The I-693 in a sealed envelope from the DCS should be submitted with the AOS submission. No physical is required.

MANY people use Dr. Arnold in Texas. He charges $30 to do the trip by mail.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

Also in our case. The Kiev medical clinic is staffed by a "panel physician" and the DS-3025 from this clinic along with a medical exam within one year should be acceptable for the AOS

You got lucky, as did my wife, but I wouldn't suggest that everybody would do the same.

Also, it sounds like they have an interview coming up and they are asking if they need to bring the form to the interview. I wouldn't bring a DS-3025 to the interview if the appt letter clearly states that they should bring an I-693. You probably sent your DS-3025 in with your AOS, correct? You didn't have to show it at the interview, did you?

Btw, every medical clinic for US immigration has a "panel physician". It's just a regular doctor who is certified by the embassy to do exams.

Q: Why are K non-immigrant visa holders asked to include a vaccination supplement when filing Form I-485 to adjust status?

Some countries allow K non-immigrant visa applicants to defer their vaccinations until after they arrive in the U.S. If the beneficiary's vaccinations were not completed as part of the visa process, it is necessary to include form I-693- part 1, part 2-vaccination section, and part 5 in the application.

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

Yes, I sent it in with the AOS. Having done three of the AOS filings myself, and seen so many others I cannot count approved the same way, I feel it was not "luck" The OP has not answered if the vaccination supplement was obtained or sent with the AOS which could have generated the request for the I-693 at the interview. If that is the case then YES they will need it.

I've always understood that if your visa medical was less than a year ago, you did not have to have another one - in which case, you'd provide the vaccination supplement. I do agree that trying to get the visa medical to stand on it's own is a bit risky. Take the short time and trouble to get the supplement completed.

The interview letter always asks for the I693 (its a standard form letter). It does not matter whether you sent the DS3025, the I693 full medical or the I693 with just the vaccination piece completed (or the I693a vaccination supplement which was last used more than a year ago).

We sent in I693a (which was still acceptable at the time of our submissions), but our interview letter said to bring a copy of the I693 medical. We went to the interview with a copy of the I693a and expected them to have and use the visa medical which was less than a year old at the time of submission. They never asked to see the vaccine supplement or anything related to the medical at the interview.

OP, you will probably be fine if you sent in the DS3025, its in your file along with the visa medical and it won't come up. In an abundance of caution, you might want to get the vaccines transcribed to the vaccine section of the I693 just in case to make sure you are prepared at the interview if they ask. If at the interview you find out that they lost your visa medical, then you will have to redo the full medical, but don't do that now.
