Is there a such form called I693a ? I only found i-693 on the uscis.gov website but not the I-693A form??

That is the vaccination supplement - usually provided by the civil surgeon.

Fiance told me he will complete all of the required vaccination before his POE on July 23.

So, would the hospital provide such documentation? Or do we have to visit a civil surgeon upon his arrival in the States and then show proof of his vaccination & then the civil surgeon will hand over the I-693 A ?

I-693A was a supplement produced by CDC that NO LONGER EXISTS, it has been replaced by a new I-693, A US Civil Surgeon only needs to fill in the vaccination section of the I-693 for K-Visa holders IF the K-visa medical is less than a year old, PER the directions included with the form.

In our case we did the vaccinations before traveling to the USA on the K-1, and had the record "Yellow Book" transferred to the I-693 by a US Civil surgeon.

It is now Part II, Item 5 of the newly redesigned I-693 that has been this way now for more than a year

But I'm still confused. Do we fill out the entire FORM of I693 for the purpose of just completing the vaccination supplement part?

Because, fiance is coming on a K1 Visa, he completed his medical in April, 2009. Should be good for 1 year..... and medical will go directly to the USCIS.... so question is do we still need to complete the I-693 FORM for our AOS ?

He is still missing 1 shot from the list of vaccination which he will complete end of this month; therefore....--> his current medical packet will state that he need ONE additional SHOT...

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

Also, it may be good for him to have full vaccination record from his doctor.

I know, we don't have family doctor concept much in South Korea.

But if he can get it, it may be good for future.

When he arrives in U.S., he can provide it to family doctor when he visit for routine check-up, and the doctor will put it in the system.

since the Ds-3025 is NOT complete and the final injection was completed later then the I-693 with Part II Item 5 filled in will need to be included to show a COMPLETE vacciantion history...

This is simple. You take the DS3025 or other proof of vaccination to a Civil Surgeon and they transcribe it to I693 part II, item 5. You have to fill in the info in part I. That's it.

Now, you could use Dr. Arnold (search the forum for Dr. Arnold) if your local civil surgeon(s) is very expensive.

A Civil Surgeon is a doc authorized to perform medicals for USCIS. You can find one in your area using the USCIS Civil Surgeon Locator

If (1) every shot was given before the medical in your country and (2)you have them all documented on the DS3025 form, and (3) they gave you a copy of that form and (4)you apply for AOS within one year of having the medical....then you can just send a photocopy of DS3025 and skip the civil surgeon part. We did, others did, and were approved without RFE or question about not having an I-693. Our medicals were after the supposed cut-off date where the new form went into effect. The RFEs you see for medicals are normally because the medical in the brown envelope turned in at POE doesn't make it to match up with the AOS file. They lose it and request a complete new medical exam in the states.
