Some are no longer in business so no address ?

i would list all even if not in business,,,maybe include pages from phone book when they where in business

On ours, both my wifes and mine, I listed the city, state and country. That is all. List the address (or city and state) of the employer when you worked for them

They just want a basic idea. Doesnt matter if the business is still active or not.
