I am a k-1 visa holder and am aware that i need to submit that vaccination supplement (I-693A) form along with my I-485 form. Do we need to do a physical exam too? Or just submit our medical vaccination form prior to coming to the US to a doctor so he can fill up the I-693A form?

I am really confused. I called so many civil surgeon clinics around San Jose and a lot of them insisted on the whole physical exam thing which cost a bomb! I already paid so much for my medical exam in Malaysia and it doesn't make sense to redo everything again. Nevertheless i found out that one clinic charges $66 dollars just to get the I-693A form filled.

Hope someone can give me some info!

I-693A is now obsolete (as of May 2008). You will have them preapre part II Item 5 of the newly redesigned I-693. If your K-1 medical was less than 1 year ago then you will not need to do that

a lot of members on visajourney, me included, thought their case would be fine since they did the physical overseas as long as it was within one year, but then at some point they get an RFE requesting for a medical examination and immunuzation.if you have a copy of vaccination, show it to a uscis designated civil surgeon .but in my opinion , it would be safer for you to redo a medical exam too.iam scheduling to redo a whole medical with immunization for next monday because of an RFE i got from california 2 days ago..but to be fair , i did not provide them with the form I-693A because i thought my medical file in the envelope i gave to the officer at the airport was going to be automatically sent to the immigtation offices.it's not always the case and alot of people have to redo everything , and also there has been a new law in effect about the physical. you may be surprised to hear you need to redo everything either from chicago, missouri or from california .these are the offices most AOS cases go though while pending for permanent residence.you may have no problem , but you may risk a delay and have to redo everything again.i wish you good luck

It seems that you are confused about the process. Let me explain. You DON'T have to re-do the medical exam if it's less than 1 year old. The reason that you got an RFE is because you did not include any medical forms with your AOS. You were wrong thinking that the medical forms that you submitted at the airport were enough. Yes, they were sent to USCIS. But besides those forms, you are also required to include an I-693 form with the AOS forms. The whole form doesn't have to be completed, just the vaccinations part and signed by a civil surgeon.

DON'T waste you money on another exam. Take your vaccination worksheet (DS-3025) that you received at your exam overseas and go to the clinic that fills out the I-693. Pay the $66 and have them transcribe the vaccinations from the worksheet onto Part II, section 5 of the form, and sign it. That's it. The other sections (medical exam) of the form DON'T need to be filled out.

yeah, for some reason the secretary of the civil surgeons don't really understand. They keep insisting i need a full physical exam. *sheesh*

Oh yeah and i have another concern. So we are supposed to be given the varicella shot. However i already had chicken pox as a baby and in my vaccination form from Malaysia it just states VH which means i have history of varicella. Is that valid? Or will they have to give me the shot? Has anyone been in this similar situation before?

Yes, or you can have them do the test to see which vaccines you already have. I think it's called a "titer test" or something like that. It's cheaper and less shots than getting the vaccines again.

I vaguely remembered reading about such a test so I asked the nurse I talked to this afternoon, and she kept repeating you need to have the vaccinations again, there is no other way. I can't believe they would actually advise getting the same shots again when there is an alternative.

Yes, it is indeed possible to go and get tested for your current immunities. David, my husband, did. No big deal! It can be dangerous as well (according to my family doctor) to get some of them again if they've already been done.

Do not let the doctor try to get you to do more than you need, a new medical is not needed if you've had your medical in the last year. It says so clearly in the instructions for the I-693. David and I paid $25 for the transcription. Call around to different Civil Surgeons or use Dr. Arnold on the first page of the I-693 info (see link below).

7/29/08 MMR shot #2 given and dropped off papers at Civil Surgeon (CS)

7/31/08 Picked up I-693, completed by local CS, (I paid $25, don't be fooled)

8/3 - 8/6/08 Worked on forms I-485, G-325A, I-864, I-765 and I-131; sent to Chicago - YESSS!

8/11/08 NOA1 for AOS, EAD and AP; 9/2/08 Transfered to CSC; 9/4/08 Biometrics

10/24/08 Rec'd AP; 10/27/08 Rec'd EAD; 11/24/08 Rec'd GC
