My vaccination supplement ( form 1-693) for a k 1 visa was completed in morocco last december as a part of the visa process, but unfortunately I did not get a copy from the physician.

Now that I intend to apply for the the adjustment of my status I-485, I am required to submit a copy of the vaccination supplement which I have taken in Morocco. But, i don't keep any copy.

I just wonder what Iam supposed to do! I think my whole medical report was in the brown envelope i submitted at the POE. Shall I try to get in touch with them? Or shall try to get in touch with the physician in Casablanca and ask them for a copy if they still keep the record? there are tow doctors working in partnership with the amercican consulate in casablanca. Can any one provide me with the phone number of the female doctor, for she was the one who made my physical examination. thank you!

I am in the same dilema so i called the immigration office yesterday. The rep on the phone told me I don't have to send copies of my medical report. You can call them and double check.

Do you have all your vaccination records? You can take them to a Civil Surgeon here in the US to have them transcribed.

If not- you can get a blood test called a Titres to check what you have immunity to.

I too did not get a copy of my vaccination record from the Doctor, but I did have my Lab results from the Titres. I brought that to the Civil Surgeon here in the States.

hi, i had my medical in casablanca last july,i went to that damn dr khadija,they didnt give me any copy or record ( i didnt ask about that as i didnt know about it at that time) ,anyway i sent the AOS last March without any vaccination thing, i just wrote in a paper that no copy or record was given to me. Till now i have not got any RFE, i hope i wont. I asked my siste to contact that Dr office ,she did , they didnt answer at first ,then when they did they said the responsible of the office is not there ,my sister asked a relative to go in person to that office,they asked him to come back,he went there again and gave him informations about me ,name ,date of the medical tests ,moroccan ID number...and they told him they dont have any record about me ( those S@@@ F@@@@ people are just playing). The same thing happened with my husband. He came on a K-1 and didn't have a copy of the vaccination records from Morocco. So we had a choice to have him re-vaccinated or have his blood checked (titres) to see if he is immuned to all of the diseases he was vaccinated for in Morocco. We decided to go to my regular doctor (luckily my insurance covered him for this) and had his blood checked for all of the required vaccines on the I-693 form. We then took all of the blood results to the civil surgeon in our area with the I-693a supplement form and he filled out the form and signed it saying my husband was properly vaccinated for all of the required diseases.

It would have been much easier (and a little cheaper) just to have a copy of the vaccination records from Morocco but this was the next best thing for us.

We didn't receive any RFE's. I hope all goes well for your AOS!

The number is 18003755283 for national customer service for Uscis. Select no 2 from the option which is missing information from the medical report, then it will ask you for some reciept no....depending on your situation it will transfer you to right person. Good luck.

salamu alaikum,

i read your thread and i feel like most Moroccan that try suffer from that problem, i went to Dr. Khadija Zerari but i have someone with me and he charge on doing everything, actually he is my friend, anyway i got my copy of vaccination document sheet, but once they check you here they also ask you to have some more shots, so i really agree with those people that advice you to take that blood test " Titres" to see what shots you need, i pay $180 for varcela shot and for the my vaccination doc. to be transcribed and add the fee of the gas trying to look for a cheap place and waste of time, it's sucks ... so you better start moving then wasting your time by calling people on the phone they will not make it any better, good luck!

Thanks for everyone. Well, I called the USCIS , and the clerk said I did not have to send that form since I did the vaccination as part of the k1 visa process as long as it was within one year .So, i suppose i just need to go ahead and send it without the form . Thanks for your help .

AMEN!

People just don't want to believe it but it is true. You don't "need" to submit the medical info to apply for AOS when you are a K1 and had a medical less than a year ago.

But hey if people really want to spend more money and like to get through the trouble it is up to them. I didn't submit any medical info with my AOS and didn't hear anything so far. As a matter of fact my case was transfered a while ago, and it seems that cases that are transfered will be approved without an interview.

Oh well, we'll see...

No, you don't need to submit a complete medical if you had one done within the past year, but if you don't have an I-693A, the vaccination supplement, you will need to prove somehow that you received vaccinations! So, sending a copy of your complete medical, might be the answer. A Titre test will also confirm that you've had the shots, or what you still need to get. If you didn't send anything with your AOS, and never get an RFE for vaccination information, then great...but I would suggest that you're very lucky if this happens. I also notice you haven't received your AOS, so I suppose you could still get an RFE.
