Hello!

I have all my paper work done for the AOS with the exception of the I693. My wife came over on a K1 visa and had the medical exam done in nov. 07. At POE Chicago they kept all her documentation so I do not have a I693 or I693A. What do I file? The only thing they let her keep was the Xray of her lungs. I currently have nothing that shows her Vacan. history and I see the I693A needs to be done by the Civil Surgeon.

3-04-09 - Called and they confirmed. Will contact me within 45 days.

3-05-09 - Date on the EAD NOA1

3-12-09 - Received a confirmation email that I am outside of the window and will get a decision in 60 days.

3-27-09 - Touch!

5-09-09 - Randomly received the renewed EAD Card... w/ wrong A# :(.

Does she have the vaccination documentation worksheet from her homecountry?

If so this can be put on the I693 form from a panel physican here.

Dr. L E Arnold in Texas does it for 10 Dollars.

You can mail him everything so you dont have to go there in Person.

If you have the DS 3025 then you have what you need... if not you need her vaccination records from her and get them transcribed by a civil surgeon...

Well it is at ones own risk,I have read of people that got denied because they did not mail it,even though the medical exam was still "valid"

Okay well now its 20 bucks,still not gonna kill ya if you want to be on the safe side.

She does not have a DS 3025 from what I saw. There is no copies of her vaccination records, and she told me they took everything at Chicago. I can't find anything about the DS 3025 on the guides. It looks close to the I693A. I'm assuming this has to be taken somewhere special to be filled out. How would the doctor she takes it to know what her records are? do they file them on some data base at the POE so the dr can pull them up?

They charged me $388.00 :cry: for varicella,TD shots, Hepatitis B & two forms of I-693. did you think they charged too much?

I've read 300-500 on the post i'm reading so your right around there. $10 isn't for the shots, its for transfering info form the one form to the other.

My problem:

I do not have any DS3025. This and the I693 were retained at the POE. So i'm clueless about what I need to submit with my AOS. I REALLY don't want to put my finace through the shots again and I really can't afford a ton more money when I already had all this done in her country. I'm assuming that everything that was collected at POE will be filed somewhere and maybe i'll just write a letter explaining why I don't have anything (collected at POE).

no that sounds about right...in GA, the cheapest i found was $295, but thats from the ones i have seen, there might be someone cheaper but idk?

They are also saying someone in TX only charges $10...that doesn't make sense to me because i dont know of any doctor to charge $10 for even one shot, i went to get a flu shot at our local health department, and they charged my $20 alone for that so they might be referring to something else, not to sure.

But we needed a civil surgeon to do the complete medical process in order to send the I-693...so in my opinion, $388 sounds about right.

WOW - that sounds like an awful large amount of money to me ... but who knows. My husband just had his 2nd MMR and the Varicella blood test and the info transferred from the DS3025 to the I693A and it was only $135 total ($40 for the blood tests - $60 for the MMR and $35 to transfer).

Just as an FYI for the Varicella .. if the immigrating person shows antibodies for varicella they are not required to have the immunization!!! The blood test is MUCH cheaper than the immunization itself -- I think I was quoted $155 for the Varicella injection - Thank God my hubby showed the antibodies so he didnt need it!

Now that I read this again if you had all those injections and the info transferred for $388 that wasnt so bad (it just seems like a lot of $$)

Sounds like you need a full medical exam.

No you wont need all the shots again,she/he will probably need an Antibody test of all the shots he/she had done.

Just make some phonecalls and see which doctor is cheaper.

I know of another person that did not submit the DS3025 nor the I693A and they did NOT receive an RFE..... I believe the forms say that you should submit this information IF NOT ALREADY submitted...... JMHO - I would submit without the the I693A and in the meantime can your fiance/wife call the hospital where she had the vaccinations and possibly have them fax the DS3025 and once you receive it you can send it to Dr. Arnold to transfer.... this way you can bring it with you to your AOS interview IF NEED BE???

As I said I know of one other person that did this and they were NEVER asked for the supplement (I693A), please keep in mind though their interview has not happened yet so it may come up then.

I would just want to get it out in the mail so you can at least start the process -- get this hassell off your chest and if you receive an RFE then give them what they want...... JMHO

Pretty sure this isn't the case as she just had the full exam in Nov in vietnam, and I read on the guides it is good for a year for the K1 visa.

I know of another person that did not submit the DS3025 nor the I693A and they did NOT receive an RFE..... Outstanding advice. I never thought of that approach. I will go ahead and continue on with the AOS paperwork assuming they already collected what they need and in the mean time I will have her call the hospital and see if they can get a copy out to the States, or if they can get one to her home address over there for her mom to send to me. The process of doing this and getting it to the states could take 2 weeks to a month depending on the mail so I would much rather wait for the RFE or interview to submit it than wait another month to submit the AOS package. And then IF they need it, its just a matter of 2-3 day mail :).
