thats whaw id recomment too! call the doctor in her home country and ask them to send /fax her ds3025. and then get that info transferred at dr. arnolds office in texas.

Does she have the vaccination documentation worksheet from her homecountry?

If so this can be put on the I693 form from a panel physican here.

Dr. L E Arnold in Texas does it for 10 Dollars.

You can mail him everything so you dont have to go there in Person.

If you have the DS 3025 then you have what you need... if not you need her vaccination records from her and get them transcribed by a civil surgeon...

BTW,

Dr Arnold is a woman and she now wants $20

They charged me $388.00 :cry: for varicella,TD shots, Hepatitis B & two forms of I-693. did you think they charged too much?

WOW - that sounds like an awful large amount of money to me ... but who knows. My husband just had his 2nd MMR and the Varicella blood test and the info transferred from the DS3025 to the I693A and it was only $135 total ($40 for the blood tests - $60 for the MMR and $35 to transfer).

Just as an FYI for the Varicella .. if the immigrating person shows antibodies for varicella they are not required to have the immunization!!! The blood test is MUCH cheaper than the immunization itself -- I think I was quoted $155 for the Varicella injection - Thank God my hubby showed the antibodies so he didnt need it!

Now that I read this again if you had all those injections and the info transferred for $388 that wasnt so bad (it just seems like a lot of $)

I did have my shot records with me & other medical records but as I said..they charged me $388 for TD, Varicella, Hepa B, & $65 to transcribed DS-3025 to I-693.

Sounds like you need a full medical exam.

No you wont need all the shots again,she/he will probably need an Antibody test of all the shots he/she had done.

Just make some phonecalls and see which doctor is cheaper.

You may need a full medical exam if you cannot find previous records.

However, I think getting the Antibody tests could be cheaper and they can fill out the vaccination supplemental form. The Antibody tests should not cost you more than 50 bucks..Good luck...

You probably wont need the full medical since you are within the 12 month timeframe, but I agree with the titre test. Might save an awful lot of running around or just have the vacciantions again.

These can be done at any Doctor, then transcribed onto the supplement.

Try sending in your AOS without the supplement but get started on organising it anyway, just in case and so that you are not held up if you do receive a RFE.

NeverSeen -- where did your fiancee have her medical exam in Vietnam? My wife had hers at the IOM office in HCMC and she was given a copy of the DS3025.
