Hi Guys and Gals,

All we have for Vaccination records for the AOS interview is the DS-3025. This is the same document that we sent in with the AOS package. Will this be good enough for the interview, or are we absolutley required to have the 693? Also, should all her vaccinations be totally up to date before the interview?

Hi, you see, I went with a Doctor authorized by immigration and he put the vaccination records in the I-693 because I didn't have my DS-3025 fill out when I went to the medical exam they didn't ask for vaccination record (only for K3 visa applicants). I sent the I-693 with the AOS package. So, my opinion is if immigration doesn't have ask for I-693 (RFE) she can go to the interview with the DS-3025 if it is complete. Supposedly, she must have had all the vaccination in order to send the AOS, if you have all of them it ins't necessary to inject the vaccination again.

I got a RFE at interview for the I693 supplement. All my vaccinations were up to date and I had only submitted the DS-3025 worksheet. I just sent it to Dr Arnold and had her copy it over and sent that back to be approved for my card.

You do have to comply with the vaccinations needed for AOS unless medically or religiously unable.

You might get away with not needing the supplement but just prepare for needing it anyway. It depends on your office and interviewer.

Where is Dr. Anorld's address and is he the only Doctor in the U.S. that can do the job? If I send it to Dr. Arnold; how long does it take to get back and can I send just a copy of the DS-3025? I was wandering if there is a Doc here in Memphis that could do the same as Dr. Arnold...??

she (dr arnold) just wants $10 for the I693a as other uscis civil surgeon try to rip you off (up to $200)

takes about a week to get it back!

If you do happen to need additional vaccinations, you wont be able to get the form done by mail. The USCIS has list of civil surgeons, just call around as to who has the best price. Make sure you indicate that you only need the vaccination supplement.

Well, we broke down and decided to just go ahead and order the I-693a from Dr. Arnold. I still am not convinced that we really need it, but better safe than sorry. My husband talked to her office and they said they usually send the I-693a back within 24 hours of receiving our request in the mail. Only $10. Seems like a good deal to me.
