Working Hours: Open to the public from 8:00 am to 12:00 noon, Monday through Friday (they open at 7:30, be there early).
Best Contact Method: It is hard to get through on the regular phone number, don’t give up, let it ring. Someone picks up eventually…. Or the 0190 number.

Packet 4 Contents (if known): - Appointment letter
- Interview and medical in Frankfurt, if not taken place yet
- Directions to the consulate (which are misleading, best to look up how to get there yourself... what is valid is that following the signs to "Palmengarten" will get you there)
- Security notice (don’t bring anything that could be used as a weapon, electronic equipment e.g. mobile phones or luggage)
- Payment instructions (80 Euro, check for current fee at www.usembassy.de/visa)
Forms to Consulate? As described in Form DSL-1076:

- a photocopy of each page of your passport
- one passport style picture (attached to Form DS 156)
- Form DSL-1076: visa instructions for fiance(e) nonimmigrant visa applicants (checklist)
- Form DS 230 part 1
- Form DS 156 (needs one passport photo attached)
- Form DS 157
- Form DS 156k (the checklist says to return this one as well – however it needs to be signed in front of a consular officer)

The checklist form is actually misleading – at the bottom of Form DSL – 1076 it says: “ You should bring all of the above pertinent documents and the application forms with you to the interview”; thus you should send back the checklist and DS 230 – as everything else is an application form or a document.
Other Forms (if any)? As described in Form DSL-1076:

- Form DS 156k (the checklist says to return this one as well – however it needs to be signed in front of a consular officer)

- Your Passport (must have at least 6 months validity beyond the issuance date of the visa) (Pls note: if yours is to expire soon – apply for a new one well ahead. Currently it takes 4 – 6 weeks to obtain a new one. Cost: €26, you’ll need 1 passport picture German style)

- Your birth certificate (in Germany you can obtain an International Birth Certificate for € 7, any further copy costs half; it makes sense to get more than one, as you will need it for AOS)

- Police Certificate (each applicant aged 16 yrs or over is required to submit a police certificate from the police authorities of each locality of the country of the applicant’s nationality or current residence where the applicant has resided for at least twelve months since attaining the age of sixteen) (Pls note: it currently takes 1 - 2 weeks to get this for Germany; it costs € 13)

- Photographs (as described in packet 3)

- Evidence of Support (as per Form OF 167 or I 134)

- Evidence of Relationship (e.g. letters, photographs or other evidence of your engagement)

- € 3.58 in stamps

If applicable:
- Court and Prison Records
- Military Records
- Marriage Certificates / divorce decree or death certificate (if previously married)
