Requirements state only "A" document, it does not ask for other copies.

I had another question.

 

As I am completing the "Affidavit for delayed registration of marriage" as you can see here:

On the first line where it says:

 

"We/I ________ do I place my married name there or my maiden name?

 

On the section 1 where it says:

 

That We/I are/am  the  applicant  for  the  delayed  registration  of  marriage  of______________________________ and ____________"full name of wife__________________.

 

Where is says "full name of wife" , do I place my married name there are my maiden name?

Married name on the Affidavit.      Full name of wife = married name.

We/I solemnly swear under penalty of law that the statements made on this report are true and correct, and the attached supporting documents are authentic; and we/I hereby set our hand/s this ___________ day of ________________________________ at ________________________________________________________________.

 

What goes on the line after the word "at"?

 

is that the year 2018? It says this day ______ of ________ at _______

 

so it seems like that should be the year but it says "at" so I wasn't sure.

I want to say after AT would be the year also.      I checked our forms from back when we filed ROM and that section is blank, we filed in person during an outreach event .. they just stamped their notary seal on and called it good.     Wish I had a good answer for you.

 

Always amazes me with so many forms and documents .. Philippine law states all legal documents will be in English .. yet they are so poorly written that no one has a clue what they are trying to say.   ...  Welcome to the Philippines

Stating it that way would make me think it would be   " this 15th day of January 2018 at 3:00 PM"   as they have other documents that require the time of day signed.

I almost think one could put almost anything after AT  .. as no one really knows what is correct.  After all ours is blank

I am trying to find out where to send the Report of Marriage. The address.

 

On this page:

it states:

 

"ONLY marriages contracted in the following places can be registered or reported at the Philippine Consulate General, Los Angeles, CA: (1) Southern California; (2)Southern Nevada; (3) Arizona; (4) New Mexico; and (5) Texas."

 

I am in Michigan and we were married in Michigan.

 

When I click the link that says:

 

"For marriages outside of its jurisdiction, the Report of Marriage must be filed with the Philippine Embassy or Consulate which exercises jurisdiction over the place of marriage. To know which Embassy or Consulate has jurisdiction, please check here ."

 

it takes me to this page:

Where you send the ROM documents depends on the state where you were married.   Use the handy locator, find the state where married, find the consulate representing that state... taadaa!

I see the consulate "sample" shows the year to be after AT  ... still it reads odd.

I sent the Report of Marriage in on January 18, 2018.

 

I live in Michigan so I sent it to this address:

 

Philippine Consulate General
122 S. Michigan Ave. Suite 1600
Chicago, IL 60603

 

That was the only address I could find so I do hope it was the correct address.

 

I received a notice from USPS that the info I sent was received.

 

I also included a self addressed stamped envelope so they could return the originals.

 

I checked the tracking number of the self addressed stamped envelope and still it has no info yet. It just says label was created.

 

1) Did I send this to the correct address?

2) How long does it take for this to be done?
