DS-3025

My husband and I are getting ready to file for his AOS from a K-1 and we realized that we were never given the actual DS-3025 that was filled out during his medical exam in Peru. He asked for a copy of the vaccines he received but it's a generic form, not the actual DS-3025. Is it necessary to send it with the AOS packet? We will try to call to see if we can get a copy of it but I'm not sure if it will be possible since he is now here and can't go to the doctors office to pick it up... should we try to get his vaccination record transcribed by a civil surgeon to send with the AOS or send it without and cross our fingers that we don't get an RFE? He's pretty sure he was given all the necessary vaccines, but he did have to go in about a month after for boosters for the MMR and Tetanus, would those have been marked as incomplete on the medical exam that went with his interview?

Are there any other Peru folks out there who did their medical with Dr Sandro Corigliano, if so were you given a copy of the DS-3025 at your medical?

His entire medical examination sheets, including DS-3025, were already submitted in his port of entry time.
(remember he handed the "mysterious brown envelope" to the border protection officer.)
USCIS has stored all his POE documents in their warehouse linked to his alien number.
So technically he doesn't need to submit DS-3025 with AOS.
However it is better idea to get a copy of DS-3025 because sometimes USCIS lose our documents.
in fact, my DS-3025 was lost and I got a "no decision made" in my interview, and the copy of DS-3025 saved me to convince them.

If you are confident that he had all the required vaccinations at the time of his K-1 medical and that there is a properly filled out DS-3025 amongst his medical documents turned in at the POE, then you can send a copy of vaccination record you were given by the panel physician and include a note about the DS-3025 being included with the rest of his medical forms in the file turned over at your POE. Then, cross your fingers and hope for no RFE. If you get one, then you can worry about finding a civil surgeon that won't rip you off and get his vaccinations transcribed to an I-693. OR, you can find a civil surgeon now and go ahead and send an I-603 with your AOS paperwork.

the hospital which performed your medical exam.

(in my case, my hospital kindly offered to me to send a new copy for free)

however a copy is a copy. USCIS don't regard a copy as an evidence.

the point they care about is if it's sealed by the trusted hospital and never tampered by you.

but you may convince them that you had completed the vaccinations at K1 time and it's filed at POE if you have a copy.

When I submitted AOS paperwork, I went ahead and assumed the USCIS had our DS-3250 (noted on cover letter). So assuming USCIS has our form DS-3250 and everthing is good (I think it is) then we dont need to submit an I-693 right?

Yes, your DS-3025 would have been with the rest of your medical documents in the sealed envelope you handed in at your POE. The POE is not who provides the copy of your DS-3025. That would have been the clinic where you had your K-1 exam done. Some countries do not get vaccinations and/or a copy of the DS-3025 though.
