    As I understand, it has to be done by a Civil Surgeon who can 'append' the 693.

    That was indeed the case. Called the only Civil Surgeon on Maui - $340 CASH. For fun, called Costco pharmacy - $56.

    Dina (and I) have an interview 12/16. The instructions say to bring form I-693. Neither of us have ever heard of this. Was that part of the brown envelope or does Dina need another exam?

    Dina (and I) have an interview on O`ahu 12/16. Hawai`i state ID expired with visa. Will passport/visa and marriage certificate be enough to pacify TSA?
