I’m currently going through my medical for the i693. Cost $450 for everything including lab work. Some places were charging $650 and I’m sure that other places were less. The place I chose worked out better because they were flexible with scheduling appointments (evening and weekends). I didn’t need any vaccinations because I had a copy of my prior vaccinations from my home country. 

There’s a chat box that appears on the USCIS website called Emma and you can request to talk to an agent directly through it. I’ve heard most people get told to wait for their bio before expediting but a few have had theirs scheduled. You’ll have to then reapply to expedite after bio as well. And you’ll need clear evidence of the emergency your filing for. Good luck.

Also, make sure you are not subject to a two-year home-country physical presence requirement. It might specify on your J-1 visa.
