Hi everyone,

If anyone can assist me with the questions below it would be greatly appreciated.

Does this have to be filled in or can it be left blank for questions 27 and 28?  I've looked at the form instructions and still not sure if it applies to me, I'm doing an AOS from a J1 visa (teacher)

27. Eligibility Category. Refer to the Who May File Form I-765 section of the Form I-765 Instructions to determine the appropriate eligibility category for this application. Enter the appropriate letter and number for your eligibility category below (for example, (a)(8), (c)(17)(iii)).

28. (c)(3)(C) STEM OPT Eligibility Category. If you entered the eligibility category (c)(3)(C) in Item Number 27., provide the information requested in Item Numbers 28.a. - 28.c.

30.a. Have you EVER been arrested for, and/or charged with, and/or convicted of any crime in any country?

I've received a speeding ticket from a sheriff and it was then lowered to faulty equipment. I paid it immediately and didn't go to court. Do I put yes or no?

Thank you in advance for helping!

Leave these blank since they don't apply to your category; your category is (c)(9)

Thank you so much for your quick reply. I am not subject to the two-year home-country physical presence requirement.
