Which Affidavit of Support form I should use from my mom? Form I-864 or Form I-864EZ or Form I-864W or Form I-864A?
Is it only one of them or two of them?

Nothing else at this point, is that correct?
And no need for Religious, School, Census, affidavits records and DNA tests, is that also correct?
