I think you should apply for citizenship and for expiredextension letter, you can schedule and infopas appointment to get a stamp on his passport in case your husband want to travel.

When my husband did his medical he had to show the interview appointment letter before they would schedule the exam. 

She should be getting the packet along with her passport and visa. 

My husband went to the vaccination center listed on the Embassay website prior to the medical exam appointment to get his vaccinations. They tested which vaccinations he needed then he brought the vaccination records with him to the medical exam. It was easier to just go to the Embassay recommended vaccination center and get it done beforehand since the medical exam would of been prolonged had he waited until the medical exam for them to pinpoint which vaccinations were needed. Made the medical exam part a smoother process. 

in my case I received emails but they also sent physical mail copies to my husband but I would get the email at least 2-3 weeks before my husband received the mail. So we had both.

As of today 04.05.17 my case is complete! called NVC to confirm and they confirmed!!!
