Worse that can happen is they cancel his visa and send him back on the next plane.

I agree with p-ana, the general rule of thumb is as much time out as in. There are other issues that a visitor can run into if they spend more than 6 months in the US which may involve the need to meet IRS tax filing requirements. So in general, as much time out as in, and no more than 6 months in the US in a tax year.

At least the same amount of time out that he was in. Since he already spent 4 months inside the US this year, if he tries again this year, cbp most likely will turn him away since it will be like he's living here (6+ months inside).
also, what kind of job and ties he has to his country that allows him to be away for so long ?
