Vaccination Supplement: argument with civil surgeon

JenT, you might have already received an answer to your question, but here is my experience. I called 4 different civil surgeons in Austin where I live, and was told that they would require another complete exam before they would fill out the I-639A....blah, blah, blah.

I told all of them that that wasn't necessary, as my husband had just had his exam the month prior by a U.S. appointed civil surgeon overseas. As soon as they said he would need to schedule an appointment to come in for another exam, I said "Thank you, but no" and hung up.

I finally found a civil surgeon who actually knew what I was talking about and said they could transfer the information from the DS-3025 to the I-693A for just $10.00! I mailed in a check with the DS-3025, and they mailed the completed form to my home within a week.

Don't pay for another exam. If they're telling you that you have to, then they don't really know the protocol.

Hey Jen,

I went to the Health Departement in Lansing, I think I only paid about $50...this was the cheapest one we found around here...and I got the right form from them!

I have the same problem here in Seattle. I couldn't find a doctor that he will do it for me easily. They want me to get another examination which i don't need it. I took my results from the hospital in my home country and i have a vaccination card that i took in turkey before i came here. I dont know if it works this time, because i dont want to pay another 200 dollars. What do you suggest? :unsure:

Hi Tracy.

I did the same thing and called most of the CS in my area (Northern California). I got the same blah blah about having to do the physical exam again, etc...

Would you happen to have the phone number and fax number of that civil surgeon who transferred the info from your DS-3025 to the I-693A form? I am very inclined to do the same thing as you did...

Here's the name and address of the Civil Surgeon I used:

Not only did I receive the I-639A filled out and signed in a sealed envelope, they also made a copy of it for my records, and mailed everything back to me directly within a week.

Maybe this will work for others. I certainly hope so!!!

ok, but what do you do if you don't have the DS-3025 but did get the medical exam? in theory you should be able to get the vaccinations and the I-639A completed without the exam, as stated by USCIS on their forms. yeah right. my fiance was told at the consulate clinic that he should not get the vaccinations until reaching the US. now all the CSs won't give him the vaccinations without the medical. what a scam.

For further clarification, we supplied the civil surgeon here in Austin with a copy of the DS-3025 that we received from the civil surgeon in Lima. I don't know how one would get an I-639A without the DS-3025...

But there's a lot I don't know!

I knew that another full medical exam was not necessary, however, just to have the vaccination supplement filled out.

Unless a CS was willing to take translated vaccination documents and transpose them to an I-693A, but it would take much less effort to transpose from a DS-3025. If you don't have a copy of the DS-3025, just phone the panel physician who completed your exam and ask them to fax a copy.

After David's panel physician exam, he needed an updated tetanus, which he elected to have done by his GP in his home town. The GP faxed the vaccination record of the updated tetaus to the PP and the PP faxed the GP the completed DS-3025. David picked it up from the office.

Aaah - thanks for the clarification. I think I got more confused because Bobogirl said the CS wouldn't give the vaccinations without also doing a medical exam (and also because we dealt with the same consulate).

basically you GOTTA get a copy at your physical. And bring your GP's vaccination record fo your shots!
