I have completed and am ready to submit Forms I-130, I-485, and I-864. When I filed Form I-485 for my wife two years ago, Form I-693 (Report of Medical Examination and Vaccination Record) was not required at that time. However, it now appears that this form is required.

I am submitting this package as a concurrent filing for my mother, who is currently in the United States on a B-2 visitor visa, which expires at the end of January 2026.

I am not submitting Form I-765, as she does not require employment authorization. I am also not submitting Form I-131, since it is not free when filed with Form I-485.

 

Could you please confirm whether this is the correct USCIS page to locate an approved civil surgeon for the medical examination?

Additionally, please let me know if any other forms are required.

Finally, could you advise which USCIS lockbox location this application package should be mailed to?

What was the intent of the tourist visa? If immigration was the intent from the beginning then that's fraud and the applicant must return for consular processing. I would file the I765 because it would be needed for social security number and state ID or driving licence.

It's a dodgy time to be trying to adjust status from a B visa.  Hopefully whatever she said at CPB won't come back to bite.  The smartest and safest would have been to do consular processing.

I totally agree. @prai, have you seen all the cases of people going for their interviews and being detained/deported because they’ve overstayed their original visa?
 

I wouldn’t risk it at the moment, if it were my mother I’d file the I-130 now but have her go home before her B expires in January and continue via consular processing.
