I'm petitioning for my wife for an IR-1. We lived in the US previously (entered on a K-1 visa and the she got a Green Card) but moved back to Italy before she was able to get citizenship.

The I-130 form asks for the "place, date of filing, and the result" of the previous petition. 

I can probably track down the date, but what are they asking for with the "place" of filing? The fields provided are only "City or Town" and "State" so apparently it needs to be a location in the US.

I would use the address where you were living back when you filed the K1.  USCIS should already have the info, so it is not that critical.

How long ago did you move back to Italy? Is her GC still valid? Did she keep any ties to the US (property, bank account, filing taxes etc)?

We were both living outside the country at the time. I was able to find our I-797C NOA with California service center address so I'm guess that would work as "place".
