3- The Question ask about to list all the beneficiary children ( My mother ) we are all adults and married .. Do I need to list myslef and brothers and sister as her children even we are all adults ?

4- Any originals would be needed to be shipped to the US here ? ( right now I have pdf scans for all the documents for my mother .

3.  Yes.  Read the questions carefully and interpret them literally.

4.  You do not need any of her original documents.

Do NOT send ANY originals to USCIS unless they ask for them.

If her name changed, yes.
