DS 260 minor child questions

Hi All,

My wife is filling out the DS-260 on behalf of her son (my stepson), who is a minor. We had a few questions -

 

- Can my wife put her contact information for phone number and email address (my stepson has neither of his own)

 

- While my stepson does not have knowledge of the name or other details of his biological father, my wife does, so can she put those details in the form as she is filling out the form on his behalf?

 

- For the vaccination question, we put "No" and added a comment "I will get all my vaccinations in accordance with US law at my medical examination prior to the interview"  - is that ok?

 

- I assume we will need to put "yes" in answer to - "Did anyone assist you in filling out this application" and put my wife's details as the preparer, is that correct?

Had a question regarding login to CEAC - does it matter whether I choose "Applicant" or "Petitioner" when logging into CEAC ?
