My fiance in Medellin looks forward to the I-129 process, but I think I need to consult an attorney because she has requested that her 23 year old daughter come over with her.  I have plenty of rooms in the house, but I told her I am not sure that she can bring another adult foreigner with her on the I-129 path.  I suppose we could marry there and proceed with a family visa, but even then.  Would I have to adopt the daughter to petition her to come as well?

A 23 year old cannot come as a derivative child on a K visa, nor can you adopt an adult.  There is no "family visa."

 

If you marry the mother, she can eventually petition the daughter (as long as the daughter remains unmarried).  It will take many years, given that  you are doing a K-1, and your fiance will not be able to petition her until she has a green card.

You are correct, she cannot bring an adult daughter with. You don’t need to adopt her and in fact can’t at this age for immigration purposes anyway.
When your fiancé eventually gets a green card, then she can sponsor her daughter. Her daughter needs to remain unmarried for this to happen as long as her mother is only a green card holder. It will probably take about 7-8 years afted her mother files the petition so, realistically, she is not looking at coming till she is in her 30s. 
If her mom /your fiancé  then wife becomes a citizen the daughter can marry without voiding the petition but then the wait time approximately doubles.
