Dual citizenship form UK/US

I had my citizenship interview today and he asked me if I had filled out my dual nationality form, which I said no I didn’t think there was one and he said I had to do it before my oath ceremony. 
 

Cab anyone help with what form this is?

There is nothing you can do.

 

US does not acknowledge your UK Citizenship UK recognizes both.

 

Wonder what he was thinking of, but only he could answer that.

I don't think there is such a form as far as US naturalization is concerned.

I suspect he was confused about the requirements of the UK in order to keep UK citizenship.

 

Some countries (eg: Germany) require you to apply to be a dual national before becoming a citizen of a new country, otherwise you lose your previous nationality. The UK has no such requirements though.
