Your fiancee and her child will need another COVID test prior to travel.  For that time, here's the list of DOH-accredited testing facilities -- https://doh.gov.ph/licensed-covid-19-testing-laboratories

 

There are 300+ facilities in the list, 100+ of which are in Metro Manila and many are privately-run.  I went to a private hospital myself and I was in and out of the testing facility in 15 minutes.

The ER fee sounds scammy but then again even the US they bill like a few thousand dollars for a COVID test in an ER vs the same test in a clinic.

I WISH I had seen this sooner.  Honestly they caught us off guard though.  She was all set to go back to SLEC ermita for her second PCR and they turned her away.  So she was in a scramble to figure out where to go.  We basically got screwed by them.

I kind of assumed that for air travel seeing as how it's been going.  When that time comes I'll just have her get a local one in her town to avoid this mess.

That's fine as long as the testing facility in her town is also DOH-accredited and is aware of the test result information that CDC requires.  From my experience, airline staff seem to be quite strict in checking compliance with the CDC's testing requirements.
