Plenty of examples floating around.  But only include them if you have no other primary evidence to include.  They are secondary evidence just like photos.  Often times including affidavits creates more issues than they solve.

Have whoever is attesting to your relationship address questions like these:

1.  Explain how you know the couple.  How long have you known Spouse #1, and how?  How long have you known Spouse #2, and how.

2.  What is your relationship to the couple?  Are you a relative?  A boss?  An advisor (like university or religious)?  A friend?

3.  How do you know the relationship is valid?  Did you attend the wedding?  Did you attend an engagement party?  Did you introduce the two spouses?

4.  If you are family, explain how the other spouse is welcomed into the family.  This is important if there are "red flags" like age differences, religious or cultural difference, same-sex marriage, etc.

As another poster said, affidavits are most useful if you do NOT have loads of financial co-mingling.  They can be very useful if there are "red flags".  They are also very useful if you do not have a lease or mortgage - such as if you live rent-free with a relative or friend.  If this is the case, the person needs to state that you live with them, what relationship you have with them (friend or family) ,whether or not you contribute to the rent or to utilities, how long you have lived with them, and how long you are expected to live with them, and why (like saving for a down payment, etc.)

Best of luck!
