I 485 interview done but they sent letter

My mom done I 485 interview but they sent letter for following evidence;

 

1 - New medical as we submitted with I 485 in 2017 which was expired!

      (this is no problem we will do again)

 

2 - Police Clearance certificate for last five years since her finger so soft due age and USCIS unable to get verification.

      any one have any idea or similar issue, how and to whom we should apply for this certificate either we apply to FBI or State both, since she is Houston, TX.

       Please advice, Thanks

USCIS requested a US police certificate? This is unusual as they perform their own checks. If they are asking for a police certificate from Pakistan then its also strange because the US does not view them as legitimate

They may have asked for a police certificate for another country that the applicant lived in
