EAD Rejection notice for eligibility category C05

Hi, 

I could really use some help here. This is the second time i'm getting a rejection notice (form I-797c) for my EAD application. 

I'm a J-2 visa holder and have received an EAD twice in the past. My eligibility category is C05 and just like in previous times I have provided the DS-2019 and all supporting documents. 

Both in the first rejection and in this one, the rejection notice simply says: "The application/petition was filed on an outdated version of this form...."

 

Now, to my understanding this could only refer to the main form I-765. Indeed when i applied the first time (sometime in July) i saw a message in the USCIS website stating there will be a new form and starting August 25 this will be the only applicable form. However, since i applied before that i used the "old" form. After I got the rejection notification, i called the USCIS and talked to a representative who heard my explanation and checked online and agreed that i acted properly and recommended that I submit again exactly the same form as the first time. Since I was afraid to get another rejection, and since this time the date was already August 22nd (very close the aforementioned August 25 deadline) I decided to fill both form: the old and the new one and attached a letter explaining that I'm only taking extra caution. Imagine my surprise when I simply got another rejection without any further explanation!

 

I really need some guidance here. I'm really frustrated that in the meanwhile i'm losing valuable work-time and money. What could i possibly be missing? is there a different form which might be outdated? I didn't see any other versions of the G-1145 and I used the latest version of the payment form G-1450. What should I do next?
