Yup thats how the wallet size looked then.  I have mine from 2000 that looks like that only the plastic outside peeled off the letters and made it unreadable. The new ones are printed on the same material as the new money.

Timeline

There are 3 types of birth documents in all of Canada outside of Quebec:

 

1. Short form, in the 90s this was wallet sized, only shows your details. 
 

2.Long form, shows you and yours parents details. 
 

3. A certified copy of birth record.  This is a photo copy of what is actually on record at the department of vital statistics certified by that agency.  


OP only has 1 and 3.  Based on the screen print from the DOS above, 2 or 3 are okay.

You need to order the long form, its called. The link above tells you what it's called in your province. In Saskatchewan, it took less than a week to get it. Good luck

I have this which has been confirmed as being acceptable by the USCIS. No need to order long form.

BC definitely has registrations of live birth considering my fiance got a copy of hers.

It's uniquely Ontario thing that you need it in place of the long form birth certificate available in the other provinces.  I've seen people on the board turned away for the wrong birth certificate from Ontario for not having the live birth registration but not a single other province saying the same thing.
