Medical and AOS- What to Submit

Hi all,

 

Sorry if this is a very basic question but I’m a bit confused about the information I’m reading online.

I’m posting in my application for AOS this week with my marriage cert, I-485 etc. 

I’m confused about what I’m meant to send regarding my medical. I did my medical in October so it’s still in date. After my K1 interview I was given the cover sheet from my medical which included my vaccine information and a CD of my x-ray. Do I need to post them for my AOS, or is there another form that I’m missing from my doctors in my home country?

You need to submit nothing as it relates to the medical as long as your vaccinations were completed abroad

Most people submit a copy of the DS3025 with the AOS just in case their original one (inside the envelope you presented upon entry) does not make it into the file.

Now I did it as I had it and figured an extra page did not matter. However, 3 years later I found out my original K1 entry envelope never made it into my file until 1 week prior to my N400 interview (yep you read it right). According to the IO that is not nearly as uncommon as one would suspect. So by including the DS3025 I got spared an RFE for the AOS. It gave me an additional 6 months on the ROC though.

I wish the IOs would get there stories straight.   I was told that as it relates  to the medical information,  the document chain of custody must be maintained or it isn't acceptable
