My fiance and I plan on doing the k1 visa, but I would like her to get vaccinated here in the USA (during AOS) rather than in her home country.

When she attends the Medical Interview before attending the interview at the US embassy, can she tell the doctor that she wants to have the vaccine in the US?

Will the doctor approve of this?

thanks

Yes, vaccinations are optional.   If you complete after the visa medical, plan on visiting a civil surgeon to complete the documentation on the i693

YMMV

so the i693 form must be completed once my fiance is in the USA? or should i complete it before she arrives?

What to complete?  That section is the civil surgeon's responsibility

YMMV

It's okay to do this (legally), but since it's usually much less expensive to get vaccinations done outside the US (if you do get them done in the US; note that you do not have to have them done by the civil surgeon; you can get them done at a clinic or somewhere covered by your medical insurance), it's not recommended unless there's some reason why it's not practical to get all required vaccinations done at or prior to the overseas medical.
