My wife recently received her EAD card, so we went to the Social Security Office to apply for a SSN card. They filed the application but said that USCIS has my wife's maiden name in the system. The EAD card has her married name on it, and everything for AOS was submitted with her new married name. They said he had to send it  back to USCIS to verify the name change. How is this possible? I did a SAVE check and there is a pending case open.

It's a manual verification and recently more rule than exception. I had the same when I requested my SSN 2 days after I moved here. The gentleman who helped me informed me that USCIS went over to a new system and that it has caused a lot of problems verifying immigration statuses, changes etc. There's nothing they can do about it, the system simply won't let them request the SSN without following this procedure.

Mine took about 4 weeks to get my initial SSN card and another 4 weeks to get the SSN card in my married name (I applied in my maiden name and when I changed my name they had to verify again).
