You just did report it. You ARE requesting a notice. You are not requesting a document. They will contact you and tell you how they will send you a new one.

The USCIS Tools  drop down boxes do not allow any information except that which concerns  the iI131, N400, N565, N600.  I tried submitting but my query was rejected because it did not match the Form # (I-485) associated with my Receipt #.

This IS the only option that SHOULD work. You did it correctly. You have a servreq#. They will contact you. If they dont you can call with your ServReq#.

This IS the correct option as you are not requesting a document, you are requesting a notice.

"If you do not receive your request for evidence by December 18, 2018, please go to www.uscis.gov/e-request to request a copy of the notice."

Notice vs Document

USCIS website is so fine-tuned to narrow the choices and restrict  interaction it confuses "We the People'.

I'll continue to show  patience and wait as I have no other options it seems.

Thanks for walking me through the USCIS Tools though,

Happy New Year
