My brother just had his Interview today (marriage-based green card AOS). The consular didn't say anything, she just gave him a paper saying that they need 120 days to take a decision.

Does this imply anything? any similar experiences.

It does not imply anything. It is normal processing time although he might know how the interview went and if there was any red flags in his case.

There was no red flag, but the officer didnt say that he is approved or anything. Would this means that he is approved, as there was no red flags?

I would imagine they are doing a background investigation.
