Form I 751 expiration date

Hi!

I am due to remove my conditions in January 2018. I downloaded the I 751 form from the USCIS site and at the right top corner of them form it is said that the form expires on 11/30/2017.

 

Does this mean that we have to wait until USCIS uploads an updated version of I751? Do we fill in the existent form?

 

Also, for those who had health insurance. Do we just make a copy of our health insurance cards and mail them or do we need some more information from the health provider?

That's the OMB expiration date for comment. It's not the expiration date of the form for public use / USCIS acceptance. You can use it fine.
