Sorry if I may sound dumb but I would rather ask than make a mistake lol...

 

Ok, I got confused! Two opposite instructions...

..."answer the questionnaire on the day of the ceremony(front page).."

..."you MUST bring the completed questionnaire to the ceremony(back page)...."

Which one?

 

Also....

Signed at_______________(city & state)........is this supposed to be the uscis address?

 

How about...

Full Address & zip code

Street # & Name

City or Town

Is this my address??

On the front page, it is just to alert you that there is a questionnaire to fill out the day of.

The second line is more or less just stressing to make sure the oath ceremony letter is brought with you on the day of your oath.

USCIS prefers you fill out the questionnaire on the same day as your ceremony day, is all.  But people fill it out beforehand all the time, too.  Just be sure if you are filling it out beforehand that nothing will change during the time between filling it out and your oath date when you hand it in to them.

 

Personally, I plan on filling it out the day of my oath ceremony---because anything can happen, like getting a traffic ticket on the way there ;)

congrats on your Oath ,I completed the questionnaire a day or 2  before the ceremony  just put the date of the Oath , 

 as for the address at ..signed at.... I put the city where the Oath took place ( but that's also my home city ) 

Full Address & zip , I put my home address  , 

 they looked it over when I handed it to the Officer and said looks good then guided me to my seat  ( we where over 140 applicants  )

I completed the questionaire 1 day before but signed and dated on the day of my oath ceremony. The Officer did ask, one by one, all the questions on the sheet to verify the answers. As to the address, we were specifically told to put the city where the oath ceremony took place, in my case, Atlanta.
